Rules & Guidelines:*

Burleson Adventist School is pleased to offer students access to the computer network resources and the Internet. To gain access to the internet, all students must obtain personal permission and must sign and return this form to the school. Parents, please read this document carefully, review its contents with your student, then sign and return to the school.

By signing this agreement, you agree to abide by the applicable BAS policies in regard to computers, computer support equipment (printers, scanners, cameras, projectors, flash drives, etc.), and the Internet. The school monitors inappropriate use of this system and makes responsible efforts to block inappropriate Internet sites but it is not possible to completely prevent access. If you accidentally access an inappropriate site, turn off the screen and contact your teacher immediately.

Rules & Guidelines
Electronic systems shall be used in support of BAS activities;
The system may be used for research and academic goals;
Access e-mail is for academic activities and is supervised by teachers.
Approved games may be played at appropriate times.
The content of electronic messages, documents, and graphic images shall conform to the ethics of the Ten Commandments and study standards as set out in our school Handbook.
All electronic systems and data are property of BAS and to be considered BAS records.
Private or personal software is not to be placed on BAS systems.
Accessing unauthorized network resources, chat rooms, and instant messaging is expressly restricted.
Each user is responsible for ALL activity performed with their assigned user ID and password.
Downloading executable programs, music, photos, or movies, without the express permission of the technology coordinator is cause for suspension.
Posting messages or accessing materials that are abusive, obscene, sexually oriented, threatening, harassing, damaging to another’s reputation, or illegal is grounds for suspension or expulsion.
Any vandalism will be grounds for disciplinary action, as well as restitution of property.

Failure to comply with these guidelines is subject to disciplinary action, up to and including expulsion.

Parent/Guardian Permission

As the parent or legal guardian, I grant permission for the above student to access the Internet as directed by a teacher. I understand that this access is designed for educational purposes. I understand Burleson Adventist School’s policy for acceptable student use of the computer, the network, and the Internet and have discussed this issue with my child. I understand that individuals and families may be held liable for any inappropriate behavior. I understand that some materials on the Internet may be objectionable and I accept responsibility to work with the school in guiding my child. I hereby give permission for my child to use the Internet and school computer system.

Technology Usage & Responsibilities

Cell Phone Liabilty Waiver

Cell Phone Turn-In Policy In accordance with Burleson Adventist School’s policy on managing student cell phone usage during school hours, all students are required to turn in their cell phones to the designated area upon arrival at school. The following waiver must be signed by both the student and a parent or guardian in order to ensure the safety and security of the device.

Consent*

1. Responsibility of the School
Burleson Adventist School will take reasonable precautions to ensure the safety and security of the cell phone while it is in the school’s possession. However, the school is not responsible for any loss, theft, or damage to the cell phone while it is being stored.
2. Acknowledgement of Risk
By signing this waiver, the parent/guardian and student acknowledge that there is a possibility of the cell phone being lost, stolen, or damaged while it is in the school’s custody. The school will make reasonable efforts to return the phone in its original condition at the end of the school day.
3. Release of Liability
The undersigned hereby release, discharge, and hold harmless Burleson Adventist School, its staff, and its administrators from any liability for any damages, loss, or theft of the cell phone during the school day. This includes, but is not limited to, damages or losses that occur through accidental causes, theft, or any other unforeseen circumstances.
4. Insurance
We recommend that parents consider securing insurance for the student’s cell phone, as the school does not provide coverage for lost, stolen, or damaged phones.
5. Return of Cell Phone
The cell phone will be returned to the student at the end of the school day or upon request. The student must pick up their phone directly from the designated area at dismissal.
6. Violations of Policy
Failure to adhere to the cell phone turn-in policy or any misconduct regarding the cell phone may result in disciplinary actions, including but not limited to detention, suspension of phone privileges, or further consequences as outlined in the school’s student handbook.

Acknowledgement and Agreement
We, the undersigned parent/guardian and student, acknowledge that we have read and understood the terms of this waiver. We give permission for the student to turn in their cell phone as outlined above and release Burleson Adventist School from liability in case of any loss, theft, or damage to the phone. The student agrees to abide by the rules regarding cell phone use and understands the consequences for violations.
